Routine Care

Our routine care provides regularly scheduled visits from our licensed nursing team to manage symptoms and ensure optimal comfort. This includes medication management and an ongoing assessment of your loved one's needs. Our nurses develop personalized care plans that honor individual preferences while maintaining clinical standards, with 24/7 availability between visits.

Continuous Care / Crisis Care

During periods of acute symptom management or medical crisis, we provide intensive, continuous care with immediate expert attention. This specialized level requires clinical approval from the Medical Director and our Nursing Team, following Medicare guidelines. Continuous care involves 8-24 hours per day of dedicated nursing presence, allowing patients to remain in their preferred environment while receiving hospital-level monitoring.

Respite Care

We recognize that caring for a loved one can be demanding for family caregivers. Our respite care provides temporary relief through short-term placement in a skilled nursing facility with 24-hour nursing support. This Medicare-approved service, available for up to five days, allows family members to rest while knowing their loved one receives professional, compassionate care throughout the hospice journey.

General In-Patient Care

When medical symptoms become too complex to manage safely at home, our general inpatient care provides hospital-based hospice services. This addresses situations where patients are medically unstable and require intensive symptom management outside a hospital environment. Limited to five days per Medicare guidelines, this service ensures acute medical needs are met while maintaining the hospice philosophy of comfort-focused care.

Our Admissions Process

For Patients with a Primary Care Physician (PCP)

When your physician recommends hospice care, you'll receive information about available programs and providers to help make an informed decision. Once you choose Amor Hospice Care as your partner, simply notify your primary care physician and provide our contact information. Your PCP will send the hospice order with clinical documentation and necessary laboratory results. Upon receiving these materials, our team immediately coordinates all admission arrangements for a smooth transition.

For Patients without a Primary Care Physician (PCP)

Some patients may not have an established primary care relationship or may be unable to visit their physician due to mobility limitations. We offer a self-referral option that empowers patients and families to elect hospice care independently. Our experienced Nurse Practitioners conduct comprehensive home evaluations, assessing hospice appropriateness according to clinical guidelines. When our NP determines a patient meets qualifications, they provide necessary medical orders to initiate services and begin providing compassionate care.

We provide hospice care to individuals in Houston and surrounding counties. Currently, we offer hospice care in Harris County, Brazoria County, Ford Bend County, Galveston County, Montgomery County, Waller County, and Wharton County.

Medicare and Medicaid cover the full cost of hospice care. If you are not currently covered by Medicare or Medicaid, but you have been diagnosed with a terminal illness, you may qualify for Medicare. Please contact our social worker or 1-800-MEDICARE to discuss your options.

Yes, we can provide 24-hour care services to patients who require around-the-clock skilled nurse monitoring. However, not all patients are eligible for this service. Due to Medicare and Medicaid restrictions, only patients who qualify after a doctor's assessment qualify for 24/7 care.

Who Pays for Hospice Care?

Most hospice patients pay nothing out-of-pocket for hospice services. Hospice care is a fully covered Medicare and Medicaid benefit, ensuring that families can focus on comfort and quality time together without financial burden. Here’s what you need to know:

If you are not currently on Medicare or Medicaid, but you have been diagnosed with a terminal illness, you may automatically qualify for Medicare if you are unable to perform the daily tasks required to take care of yourself. Please call 1-800-MEDICARE or 1-800-633-4227
